MALPRAXIS Is Bringing Its Surgical Horror Experience on PC in Early 2026

Sunscorched Studios, the team behind the cult hit RAT SHAKERhas announced MALPRAXIS, a first-person surgical horror game coming to PC via Steam and Epic Games Store in early 2026. The game takes place on board the TRH Rusanov spaceship. The game plunges players into a clinical nightmare where survival depends on precision, judgment and restraint.

In the game, players take on the role of a surgical emergency response unit, a diagnostic known as SPIDER. This is an autonomous medical system powered by a central biological processor. Over a series of semi-procedural shifts, players examine patients, diagnose conditions, perform complex surgical procedures, and conduct forensic examinations as the situation on board the ship steadily worsens. Every decision carries weight, and even small mistakes can have long-term consequences.

Your goal is tough and uncompromising, including treating patients and keeping your team alive. Every result is documented. When failure exceeds acceptable limits, the system stops the test and resumes it, erasing the memory. However, death remains constant. Crew members who die during a shift remain dead, and those losses carry over to future shifts, changing the course of each test.

One test is conducted over consecutive shifts and continues until the system reaches the failure threshold or declares the evaluation complete. This continuous cycle defines the player's reality aboard the Rusanov.

MALPRAXIS is a complete standalone game that reflects years of experimentation with medical gameplay, long-term effects and psychological pressure. The developers explored the core ideas in an earlier free demo, which was later reworked and expanded into a significantly different experience. The game contains graphic and disturbing content and is not intended to be a realistic or educational medical simulation.
